On Your First Visit

We are happy to take care of all of your dental needs here at Courtney Mitchell III Family Dentistry. In order for us to provide you with the best possible care, please bring the following with you to your first visit:

• Insurance card and information
• Personal identification (driver’s license)
• List of current over-the-counter (OTC) and prescription medications that you are taking, including dose and frequency
  (please bring medications with you if possible)
• Important medical and surgical history
